Sharing the Care Campaign of Northern Colorado focuses on patient, primary care provider specialist, pharmacist, family and caregiver issues and concerns with particular attention given to older adults. The campaign strives to identify and respect informed choices about medical care, explores the wisest practices in medication reconciliation and chronic condition management and ways to communicate and exchange essential information at every transition.
Campaign members, at a recent gathering, presented information and updates about opportunities on the Leadership Team, a soon to be unveiled Patient Passport program, Advance Care Planning, Community Outreach/Marketing and advocating on behalf of patients.
Attendees were invited to pick up for their reference and files printed information including Don’t Take the Bait – Known Scams, Mandatory Reporting of At-Risk Elder/ Adult Mistreatment, Selecting Your Own Healthcare Agent and Durable Medical Power of Attorney Forms. These forms and others, assist in navigating the medical health care system.
