Independence – this is one of those things we take for granted until we are faced with the health care challenges that present themselves with age or a specific diagnosis. At HealthSouth Rehabilitation Hospital of Littleton, we understand that rehabilitation can help keep patients independent and safe in their own home.
Many conditions are accepted by HealthSouth’s home referral program for admission, including general debilitation with limited mobility and/or deficits in self-care activities, such as bathing, dressing, or eating. Other possible diagnosis include Parkinson’s disease, Multiple Sclerosis, history of a stroke, brain injury, amputation, cardiac surgery, neurological disorders, orthopedic surgery, rheumatoid disease, and a back, pelvis or lower extremity fracture.
Patients are not required to have had a three-day acute hospital stay. So, if you meet all of the above criteria, a home admission is certainly within reach.
To begin the process, a rehabilitation liaison evaluates a patient at home via medical record review and patient/family interviews. Next, the HealthSouth medical director reviews the evaluation and determines if the patient is appropriate for admission.
Once accepted into HealthSouth’s inpatient acute rehabilitation program, a team of specialized therapists, nurses and rehabilitation physicians create a therapy plan that focuses on helping you achieve your goals. This includes at least three hours of physical, occupational and/or speech therapies five days a week, and daily physician visits. Our goal is for all of our to regain their independence and be successful living in the comfort of their own home. The average length of stay in our acute rehabilitation hospital is 12 days.
