Got a major surgery coming up? Anticipate needing a temporary or short term disability placard for parking? Don’t wait until after your procedure to start the paperwork.
Residents statewide can now complete an online application and have their temporary disability parking placard mailed to them. This saves time and a car trip.
To apply, first print and have your doctor’s office complete the Persons with Disabilities Parking Privileges Application – Form DR 2219, available for download at www.BoulderCounty.org/MV under Disability Parking. If you do not have access to a printer, your doctor’s office may have copies of this form. Check with them first or direct them to the website. You will then need to complete the top portion of the form regarding valid identification such as a Colorado Drivers License or ID.
Next, either scan the application into a PDF or take photos of the individual application pages (pages 4, 5, and 6). Visit the State’s DMV website: MyDMV.Colorado.gov and apply for a first-time disability placard under Vehicle Services. The application will allow you to upload multiple pages as needed. Once submitted online, temporary or short-term (90-days) disability placards are typically received within two weeks.
